In exchange for the participation in the activity of Swimming Lessons/Events organized by Swim2LiveLive2Swim Swim School, located at 1359 E Eagle Nest Terrace, Mustang, Oklahoma 73064, and/or use of the property, facilities, and services of Swim2LiveLive2Swim, I agree for myself and for the member of my family listed below to the following:
1.) AGREEMENT TO FOLLOW DIRECTIONS: I agree to observe and obey all posted rules and warnings, and further agree to follow any oral instructions or directions given by Swim2LiveLive2Swim, or the employees, representatives, or agents of Swim2LiveLive2Swim.
2.) ASSUMPTION OF RISK AND RELEASE: I recognize that there are certain inherent risks associated with the above described activity, and I assume full responsibility for personal injury to myself and the members of my family listed as such below and further release and discharge Swim2LiveLive2Swim for injury, loss or damage arising out of my or my family’s use of our presence upon the facilities of Swim2LiveLive2Swim, whether caused by the fault of myself, my family, Swim2LiveLive2Swim or other third parties.
3.) INDEMNIFICATION: I agree to indemnify and defend Swim2LiveLive2Swim against all claims, causes of action, damages, judgements, costs or expenses, including attorney fees and other litigation costs, which may in any way arise from my or my family’s use of our presence upon the facilities of Swim2LiveLive2Swim.
consent to the participation in the activity of Swimming Lessons and agree on behalf of the above minor(s) to all of the terms and conditions of the Agreement. By signing the Release of Liability. I represent that I have legal authority over the custody of the above-mentioned minor(s).
